Dear Mr. Tsukazaki:
This is a Notice of Claim against Louanne Kam, Kamehameha Schools; Matt Tsukazaki, Esq.; and Torkildson, Katz, Fonseca, Moore & Hetherington for breach of contract, racketeering, fraud, tax fraud, conspiracy to commit fraud, and other wrongful acts.
I am sending this Notice of Claim to your attention in accordance with the instructions in your letter of February 23, 2004. However, as explained in my letter to you dated February 25, 2004, since you are not the authorized claims adjuster for any insurance carrier, I am unwilling to provide any further details other than those stated in the first paragraph.
Therefore, I ask that you immediately report this claim to both Kamehameha Schools’ and Torkildson, Katz, Fonseca, Moore & Hetherington’s professional liability insurance carriers and have them acknowledge receipt of this notice within ten (10) days from the date of this letter.
