This is to inform you that I have not received a response to my Notice of Claim letter to you dated December 13, 2004, either from your office or from your Professional Liability Insurance carrier. Will you please have an authorized claims representative from your carrier contact me as soon as possible.

I have received an e-mail on February 1, 2005, from John M. Bishop, Vice President of your Western Case Management Center, regarding my copying of correspondence to their office. For your information, I am including a copy of his e-mail, and my response, with this letter.
