RE: IRS Forms W-2 and 1099-R for Year 2000
Dear Trustees:
This is in follow-up to my letter dated November 4, 2002, addressed to the Trustees of Kamehameha Schools c/o Matt A. Tsukazaki, Esq., Torkildson, Katz, Fonseca, Jaffe, Moore & Hetherington, regarding the failure of Kamehameha Schools to provide to me the required IRS Forms W-2 and 1099-R for the year 2000.
It has now been over seven years since these two required forms were due and neither have been received. Furthermore, your attorney, Matt Tsukazaki, has not provided any satisfactory answer as to why they have not been provided. Due to the failure of Kamehameha Schools to withhold the proper Social Security taxes in 2000, and to deposit the required amounts into my Retirement Plan account, I have not received my proper Social Security payments or retirement benefits due for these past seven years.
Therefore, I am again requesting that Kamehameha Schools immediately issue the required IRS Forms W-2 and 1099-R for the year 2000, and provide an accounting for, and payment of, all Social Security and retirement benefits due for this period of time.
