RE: Notice of Claim Against Dr. Hamilton McCubbin
Dear Mr. Tsukazaki:
This is a Notice of Claim against Dr. Hamilton McCubbin, former CEO of Kamehameha Schools, for breach of contract, racketeering, fraud, tax fraud, conspiracy to commit fraud, and other wrongful acts.
I am sending this Notice of Claim to your attention in accordance with the instructions in your letter of February 23, 2004. However, as explained in my letter to you dated February 25, 2004, since you are not the authorized claims adjuster for any insurance carrier that Kamehameha Schools may have had at any time in the past or present, I am unwilling to provide any further details other than those stated in the first paragraph.
Therefore, I ask that you immediately report this claim to Kamehameha Schools’ professional liability insurance carrier and have them acknowledge receipt of this notice within ten (10) days from the date of this letter.
