RE: Bad Faith Claim Against P&C Insurance Company
First Notice of Claim: August 30, 2006
Dear Ms. Clarke:
As P&C Insurance Company, Inc. has failed to respond to my Notice of Claim dated August 30, 2006, I am hereby giving you a new Notice of Claim for this latest failure to respond in good faith to a legitimate claim. Please note that this “bad faith” claim is separate from my previous claims of fraud, racketeering and malpractice, for which I am still awaiting a response.
For this latest act of bad faith, I am asking for the additional sum of Five Hundred Thousand Dollars ($500,000.00) from you, as an Officer of the company; plus the sum of Five Hundred Thousand Dollars ($500,000.00) from Louanne Kam, as an officer of the company; plus the sum of One Hundred Thousand Dollars ($100,000.00) for each individual who has served as an officer or director from January 1, 1997 to the present.
I also ask that you immediately acknowledge receipt of this Notice of Claim. When I am contacted by your authorized Claims Adjuster, I will provide him/her with further information, including a password to access additional claims information located at:

Any failure to respond to this Notice of Claim, will be considered another act of bad faith and will result in further demands for damages. Your cooperation in this matter will be greatly appreciated.
