Dear Ms. King:
As your company has failed to promptly and properly respond to my previous Notices of Claims against your insureds, Ayabe, Chong, Nishimoto, Sia & Nakamura, I am hereby filing this Notice of Claim against St. Paul Fire and Marine Insurance Company for bad faith in the handling of the subject claim.

For the failure by your company to act in good faith in the adjustment of this claim, I am asking for the sum of One Million Dollars ($1,000,000.00). In addition, for my original claims of professional malpractice against your insureds, I am asking for the sum of Five Hundred Thousand Dollars ($500,000.00) from Jeffrey Sia; plus Five Hundred Thousand Dollars ($500,000.00) from Sidney Ayabe; plus one million dollars ($1,000,000.00) from the law firm of Ayabe, Chong, Nishimoto, Sia & Nakamura.
Any failure to respond to this Notice of Claim, will be considered another act of bad faith and will result in further claims against you and your insureds.
Your immediate response to this Notice of Claim is requested.
