Dear Mr. Tsukazaki:
As you, and your professional liability insurance company, have failed to respond to my prior reports of claims, I am hereby sending you this NEW Notice of Claim against you, Robert Katz, Esq., your law firm, Torkildson Katz Fonseca Moore & Hetherington, and your undisclosed professional liability insurance carrier(s) for racketeering, fraud, tax evasion, obstruction of justice, destruction of evidence, and bad faith due to the failure to respond to my prior Notices of Claim dated March 4, 2005 and August 29, 2006.
Due to your repeated failure to respond in a timely manner to my legitimate Notices of Claim, I am increasing my demand for damages to the sum of one million dollars ($1,000,000.00) from you, as an individual; plus the sum of one million dollars ($1,000,000.00) from Robert Katz, as an individual; plus two million dollars ($2,000,000) from your law firm, Torkildson Katz Fonseca Moore & Hetherington; plus Five Million Dollars ($5,000,000), from your unnamed professional liability insurance carrier(s).

Mr. Tsukazaki, as you have not disclosed the names of your insurance carrier(s), I am directing this Notice of Claim to you with the demand that you submit this NEW CLAIM immediately to your insurance carrier(s) and request that a licensed claims adjuster, or their attorney, contact me as soon as possible, but in no event later than 15 days.
I also ask that you immediately acknowledge receipt of this new Notice of Claim, and advise me of the date when you submitted this Notice to your insurance agent and/or insurance carrier(s). Any failure of you, and/or your insurance agent and/or carrier(s), to respond to this Notice of Claim, will be considered yet another act of bad faith and will result in further claims.
